Follicular unit
The natural anatomical bundle in which scalp hair grows — one to four terminal hairs with shared supporting structures. Modern transplantation moves intact follicular units rather than individual hairs.
The follicular unit is the natural packet in which scalp hair grows: one to four terminal hairs emerging together, sharing sebaceous glands, an erector muscle and a fine collagen sheath. It was characterised histologically in the 1980s, and its surgical significance followed: hair does not grow as evenly spaced single shafts, so restoring it one hair at a time — or in the large multi-unit plugs of early surgery — both produce patterns nature never makes.
Modern transplantation moves intact units. Keeping the bundle whole preserves the anatomy that supports the hairs and reproduces the grouping the eye expects; splitting units to inflate graft counts damages both. Units differ in composition — singles, doubles, triples and the occasional four-hair unit — and that mix varies by scalp region and by patient, which is why the sorting bench matters: extracted grafts are classified under magnification so each class can be placed where it belongs.
Composition drives the visual arithmetic of a result. A thousand grafts may carry anywhere from under two thousand to well over two and a half thousand hairs depending on the donor’s mix, and hair calibre multiplies the effect — thick-calibre doubles cover what fine singles cannot. This is why identical graft counts produce visibly different results, why hairs-per-graft belongs in surgical documentation, and why quotes expressed only in grafts tell half the story.
Placement follows composition: single-hair units build the soft transition zone of the hairline, where a double reads instantly as a clump; multi-hair units build density behind it, where singles would be wasted. Respecting that mapping is one of the quiet disciplines separating natural results from technically successful but odd-looking ones.
The unit also sets the limits of the operation. Donor capacity is counted in units, not hairs; extraction removes whole units forever; and no instrument clinically subdivides or multiplies them — claims implying otherwise describe wishes. The follicular unit is the currency, the constraint and the pattern template of the entire field.
